I really wish I could just fill it out online!
Also, in the PDF version your title is cut off a bit, the W in ‘Web’ at the top is off the left side of the page.

@Robert: you can fill it out online, just make sure you wipe off your monitor screen when done

I agree, it would be so much better to do it online, expecially when you are targeting us web developers.
